Programmes - Outreach Programme
The Outreach Programme was founded in 2000. The aim of the programme is to provide a service to individuals within their own home or in their community, the emphasis being on designing a programme that meets the needs of the individual.
The service may consist of weekly visits to the individual's home, outings to the shops, restaurant, and leisure activities such as swimming, bowling or going to a concert or cinema.
The Outreach worker liaises with the interdisciplinary team, i.e. Psychologist, Social Work, Occupational Therapist, Dietician and other relevant professionals. The programme is reviewed on a quarterly basis by the team. Links are maintained with the services in the community such as the Irish Wheelchair Association, the Department of Social Community and Family Affairs, the local Health Board, the County Council, etc, as the need arises.
